Professional referrals in five simple steps
Find a care home
Search for a suitable care home for your client or patient by using our care home finder at the top of this page.
Contact the care home
Contact the care home directly by phone or submitting an online enquiry form to check availability and arrange a show round. You can find the contact details on each care home webpage.
Care assessment
If the home has availability to meet your client or patient's care needs, they will arrange an assessment. You will need to provide a copy of their care plan, as well as any other relevant documentation.
Reviewing your referral
The care home team will review your referral and contact you within two office days to discuss the next steps.
Admission
If a place can be offered, the care home will support you to complete any necessary application forms and financial assessments. Top-up fees can be arranged directly with the family.
