Contact the care home

Get in touch with the care home directly by phone or by submitting an online enquiry form to check availability. You’ll find the contact details on each care home’s webpage.

Reviewing your referral

The care home team will review your referral and get in touch with you, usually on the same day, to discuss and confirm the next steps.

Care assessment

If the home has availability to meet your client’s or patient’s care needs, they will arrange an assessment. You’ll need to provide a copy of their care plan along with any other relevant documentation such as a financial assessment.

Admission

If a place can be offered, the care home will support you in completing any necessary application forms. 

Our dedicated team will work closely with you and your client or patient to make the move as smooth as possible, ensuring they feel comfortable and supported from day one. We look forward to welcoming them to their new home and helping them settle in as part of our warm and friendly community.
